The Administration for Community Living, in partnership with the CDC, has invited Trellis to join the newly established Community Care Hub National Learning Community.
Trellis is one of 58 organizations across the nation that are working to connect more individuals to coordinated supports that address the social drivers of health.
In recognition of the leadership role that Trellis has played in this arena, we have been asked to present our Juniper network approach at the inaugural meeting of the learning community on November 16.
“This is a tremendous opportunity,” said Mark Cullen, Vice President of Strategy and Business Development for Trellis. “We are excited to share the successes we have had in Minnesota and to learn from others in this cohort.”
Juniper is a Community Care Hub
A Community Care Hub is a community-focused entity that organizes and supports a network of community-based organizations providing services to address health-related social needs. In Minnesota, Trellis has created the Juniper network as a Community Care Hub. It provides centralized administrative functions and operational infrastructure including contracting with healthcare organizations, payment operations, management of referrals, service delivery fidelity and compliance, technology, information security, data collection and reporting.
Community Care Hubs play an important role in promoting an equitable health and social care ecosystem. They serve as the bridge between community-based organizations, healthcare payers and providers, and public health systems to increase a community’s capacity to reach populations who are underserved.
