Personalized support for dementia care—at no cost

The Care Ecosystem at Trellis is a free, phone-based program designed to help people living with dementia and their caregivers navigate care and supportive resources. Local care team navigators (CTNs), in partnership with a licensed social worker, offer personalized guidance and connections to resources like food delivery, transportation, financial assistance, support groups and more. The team can help: 

  • Determine immediate needs 
  • Assist with medication review and navigation support 
  • Support behavior changes  
  • Provide safety screenings and recommendations 
  • Support caregiver health and well-being 
  • Share education resources and community services 
  • Navigate advance care planning

A trusted path to dementia support

We know that every dementia and caregiving journey is unique, and that’s why the Care Ecosystem program is tailored to each individual.  

We offer in-depth care navigation for up to one year and short-term “gateway navigation” if needing to address a specific concern or challenge. Our CTNs can provide personalized no-cost support, helping caregivers and their loved ones live well at home. The Care Ecosystem program model has been shown to: 

  • Reduce trips to the emergency room and hospital
  • Address medication issues and help prevent avoidable crises
  • Ease stress, with personalized support for caregivers

Make a referral

Whether you’re a healthcare professional, community organization, family member, or friend, making a referral—or self-referral—is easy. Just select the “submit a referral” button below. A representative will follow up within two business days with a brief consultation to determine the appropriate level of care navigation.

The Care Ecosystem dementia support program requires specific eligibility criteria:

The person you are referring needs to meet all three requirements below:

  1. Is the primary caregiver (18+) for a person living with dementia, OR is a person living alone with dementia – no formal diagnosis required.
  2. Lives in the Twin Cities seven-county metro community (Anoka, Carver, Dakota, Hennepin, Ramsey, Scott or Washington county). The program is unable to accommodate those living outside the Twin Cities metro area at this time.
  3. Resides in a home setting (private residence). The program is unable to accommodate those living in residential care or nursing care environments at this time.

Submit a referral by clicking the button below. By doing so, you confirm that the person being referred (yourself or someone else) has given consent to share their information and understands that Trellis Care Ecosystem will contact them to offer support or services pending eligibility review.
