The Care Ecosystem at Trellis is a free, phone-based program designed to help people living with dementia and their caregivers navigate care and supportive resources. Local care team navigators (CTNs), in partnership with a licensed social worker, offer personalized guidance and connections to resources like food delivery, transportation, financial assistance, support groups and more.
