Minnesota Aging Pathways

Minnesota Aging Pathways is a free service of the Minnesota Board on Aging, delivered by Trellis and other Minnesota Area Agencies on Aging. If you have questions about Medicare, housing, financial support or services that help you live safely in your home, we can help.

Minnesota Aging Pathways services are provided by a dedicated staff of social workers, nurses and other professionals who work diligently to understand each person’s unique situation and find effective solutions.

How can I continue to live in my home for as long as possible?

When it becomes more difficult to stay in your home, a Minnesota Aging Pathways housing specialist will help you:

  • Evaluate your current situation
  • Find services that may be helpful
  • Help you reduce the need for nursing facility care
  • Learn about financing options and other resources
How do I get long-term care options counseling?

If you plan a move into a setting that is “registered housing with services” (including assisted living), state law requires that you receive long-term care options counseling. Minnesota Aging Pathways will provide a phone consultation to ensure that your rights to choose your housing are protected. Learn more by calling 800-333-2433 and press “option 1” for housing.

I’m new to Medicare. How can I get help to understand my options?

Minnesota Aging Pathways provides free counseling to people who are eligible for Medicare benefits. You’ll understand what services are covered under Medicare, along with your options for supplemental insurance. Minnesota Aging Pathways specialists are trained and certified by the State of Minnesota.

I need help paying for my prescriptions. What help is available?

Minnesota Aging Pathways can help you explore options to lower the costs of your prescriptions, no matter what your age or situation. There are a number of programs available and everyone’s situation is different. For personalized assistance, call us at 800-333-2433 and press “option 2”.

I have very little income. Do I qualify for help with long-term care?

County social service offices and managed care organizations determine eligibility for publicly funded long-term care services such as at-home care or managed care. Minnesota Aging Pathways can refer you to organizations that can help answer your questions.
