Live Well at Home and Dementia Grant RFPs Available

The State of Minnesota has published two grant opportunities for services and supports that help people age well at home.

Live Well at Home Grants

The Minnesota Department of Human Services is soliciting applicants for Live Well at Home Grants. Successful applicants will develop or provide services that help older Minnesotans to live in the community of their choice, improving their community’s capacity to provide home and community-based services.

The funding is anticipated to be for the period of July 1, 2022, to June 30, 2023, with the option to potentially extend beyond that date. The maximum initial award per year is $350,000. There is a dollar-for-dollar or 50 percent matching requirement.

Proposals are due by 4:00 p.m. CT on Friday, April 1, 2022. For grant details, visit the Live Well at Home website. DHS will host a WebEx Responders Conference on February 22, 2022, at 1:00 p.m. CT.

Dementia Grants

The Minnesota Board on Aging has published a Request for Proposals for Dementia Grants. The grants are for regional and local projects to increase awareness of Alzheimer’s disease and other dementias, increase the rate of cognitive testing, promote the benefits of early identification, and/or connect family, friends and neighbor caregiving for persons with dementia to education and resources.

The funding is anticipated to be for the period of July 1, 2022, to June 30, 2023. The maximum initial award per year is $150,000. There is a twenty-five percent matching requirement on estimated budgets between $50,000 and $150,000. No match is required for budgets less than $50,000.

Proposals are due by 4:00 p.m. CT on Friday, March 25, 2022. For grant details, visit the MBA Dementia Grant Website. 

Technical Support

Trellis provides technical assistance for applicants for these RFPs on a first-come, first-served basis. Our ElderCare Development Partnership staff will consult about project needs and design, outcomes, sustainability and budget, and can review draft proposals as time permits. Applicants are strongly encouraged to contact Diane Graham-Raff as soon as possible to receive consultation.
