Trellis, through its Juniper program, seeks an experienced consultant to develop and deliver training for community-based organizations (CBOs) who deliver social care services that address health related social needs. Training will cover current trends in healthcare enabling payment models for social care services, business incentives in health care and how they align healthcare systems and health plans to work with CBOs, and financial and programmatic reasons why a CBO would want to offer social care services as a contracted health care benefit.
