Trellis has built the technology, infrastructure and networks needed to partner with healthcare organizations as payors for services for older adults. The result: Older adults gain to access better preventative care and supportive services that allow them to live healthy and connected lives in their communities.
We have active partnerships with healthcare systems across Minnesota as part of our Juniper® program. We are ready to engage our networks to address the needs of high-cost, high-need older adults through Medicare, Medicaid or other health insurance programs.
Part of the national Community Care Hub
Trellis is one of 58 organizations nationwide that has been selected to participate in the Community Care Hub National Learning Community by the Administration for Community Living (ACL) and the CDC.

Juniper network takes health promotion to scale

Juniper uses a network approach to disseminate evidence-based health promotion classes across Minnesota. Juniper contracts with community-based and healthcare organizations to offer education led by Juniper-certified class leaders.
Participants can easily find and register for classes using the Juniper website. A sophisticated backend management information system (MIS) allows community providers to schedule and manage classes and report participant results. The MIS makes creating financial relationships with third-party payors seamless and scalable.
Integrating health
and social care
For optimal well-being, an individual in precarious health needs access to a broad range of supports. Integrating services such as home-delivered meals, transportation and nonmedical home care with medical care improve health outcomes and can help control spending in healthcare and long-term care.
Trellis is collaborating with health systems and community organizations to build an easy-to-access gateway to integrate services that will improve older adults and their caregivers’ independence, health and well-being. Contact us to learn more about this emerging initiative.
