We are proud of our accomplishments in 2021 — despite challenges related to COVID-19. We’re sharing these highlights as a way of helping you to get to know us better.
It has been our pleasure to serve older adults and caregivers in our community this year and help build a strong network of community organizations to support the aging population. A big thanks to our providers and partners who made so much of this work possible.

Key accomplishments
- Rebranded with a new name and new look.
- Issued a revised RFP for Title III services in response to our Equity Assessment of Access to Title III Funding that made application more accessible to organizations serving BIPOC communities.
- Assisted 33,609 callers, conducted 31,273 preadmission screenings, made 142 presentations and managed 4,559 referrals to Return to Community in our work with the Minnesota Senior LinkAge Line®.
- Organized 43 COVID-19 vaccine clinics in community settings, serving 11,300 people.
