Trellis 2021 year in review

We are proud of our accomplishments in 2021 — despite challenges related to COVID-19. We’re sharing these highlights as a way of helping you to get to know us better.

It has been our pleasure to serve older adults and caregivers in our community this year and help build a strong network of community organizations to support the aging population. A big thanks to our providers and partners who made so much of this work possible.

Thank you to our tremendous staff for their work in furthering our mission of helping people optimize well-being as they age. 

Key accomplishments

  • Rebranded with a new name and new look.
  • Issued a revised RFP for Title III services in response to our Equity Assessment of Access to Title III Funding that made application more accessible to organizations serving BIPOC communities.
  • Assisted 33,609 callers, conducted 31,273 preadmission screenings, made 142 presentations and managed 4,559 referrals to Return to Community in our work with the Minnesota Senior LinkAge Line®.
  • Organized 43 COVID-19 vaccine clinics in community settings, serving 11,300 people.
